Coryell Health Clinic

LVN - FT

Summary: Performs assigned nursing duties, including assessing, planning, implementing and evaluating patient care, under the direct supervision of an RN/Physician/Midlevel Provider.  Also performs a wide variety of patient care activities including, but not limited to, maintenance of clinical record/documentation, administering medications, giving injections, and taking vital signs.

Qualifications:

1. Ability to facilitate communication and problem solving with patients, families, staff physicians, and administration.
2. Basic Life Support
3. Licensed in the State of Texas as a Licensed Vocational Nurse or possesses a temporary permit to practice vocational nursing in Texas. Nurse with temporary permit (GVN) has 75 days to take the NCLEX after they receive the Authorization to Test per Texas Board of Nursing. If GVN fails NCLEX the temporary permit is revoked by the Texas Board of Nursing and job status at Coryell Medical Clinic will be demoted to position and pay prior to GVN status (nurse tech/CNA) or may result in termination.
4. Prefer one year or more of nursing clinical experience.

Registration Clerk - FT                 

Summary:  Receives visitors/patients, determining their needs politely and directs them accordingly.  Schedules appointments, receives payments, and collects co-pays and issues receipts as applicable.  Enters appropriate data into the computer. 

Duties and Responsibilities
1. Greets visitors/patients upon arrival and proceed to check-in.
2. Verify insurance benefits as needed.
3. Accurately registers patient’s demographic information, emergency contacts, and patient/guarantor relationships.
4. Verifies and updates patient/guarantor demographic information and insurance.
5. Collects and records co-pays and payments at time of check-in.
6. Reconcile daily receipts.

8. Take and send messages to appropriate provider.

Home Health 

RN Casemanager - FT

Summary: The Registered Nurse (RN) is a professional member of the health care team who provides skilled nursing care to patients in compliance with the Nurse Practice Act and within the policies and procedures of the agency, upholding standards of nursing practice.  Keeps abreast of current techniques and innovations to meet special needs of the patients and staff.  Is responsible for monitoring, altering and coordinating the services the patient receives to promote optimum care. Is responsible for completion of all required documentation. Takes after hours, on-call responsibilities as assigned.  Coordinates schedules/staffing.  Supervises skilled and non-skilled care.  Completes comprehensive chart reviews on active and discharged patient record components for qualitative analysis of content.

Social Worker/Care Transition Coordinator - FT

Summary: Provides administrative direction to hospital and clinic social services. Develops procedures to implement policies established by Coryell Health Governing Board. Assures compliance with regulatory bodies. Communicates and facilitates service coordination and linkage for inpatients and outpatients.

Duties:

2. Communication with all hospital inpatients to verify discharge needs and follow up regarding discharge planning.
3. Work with the Discharge Planning Team
4. Working closely with physicians, Physical Therapy staff and nursing staff regarding patients’ physical status and needs both inpatient and outpatient.
5. Coordinate weekly discharge planning meetings and documenting appropriate reflections of patients’ needs and plans.
6. Faxing information such as History and Physical, medication list, Face Sheet, etc. to referral recipients on patients’ behalf.
7. Completing inpatient psychosocial evaluations for Extended Care patients.
8. Assist with completion of advance directives as requested by patients.
9. Manage patient care for those involved in the Tele-Psych Program.

Material Management

Housekeeping Personnel - Full Time

Summary: The primary purpose of your job position is to perform the day-to-day activities of the Housekeeping Department in accordance with current federal, state, and local standards, guidelines and regulations governing our facility, and as may be directed by the Administrator, and/or the Director of Housekeeping, to assure that our facility is maintained in a clean, safe, and comfortable manner.

ICU RN - FT Days, Nights and PRN

Summary: The RN is primarily responsible for utilizing the nursing process and emergency nursing standards in providing health needs to the patient in the Emergency Department.

Qualifications:

1. Licensed and currently registered as a Professional Nurse in the state of Texas or possesses temporary permit to practice professional nursing in the State of Texas while awaiting receipt of license.
2. Required to meet qualification criteria during hospital and nursing unit orientation periods.
3. Prefer one year of professional nursing experience in emergency or critical care clinical setting.
4. Require current CPR (ACLS certification within one year) and must maintain without lapse.
5. Must have PALS within one year of employment and must maintain without lapse.
6. Must have NIHSS certification within six months of hire and maintain without lapse.
7. Must have TNCC certification within eighteen months of hire and must maintain without lapse.
8. Must have basic EKG and/or arrhythmia course within six months of employment.
9. May satisfy requirements with documentation from previous experience.

ED/ER Tech - FT

Summary: The ED Tech is primary responsible for caring and treating patients in a manner conductive to their safety and comfort; performing duties in accordance with established methods, techniques, and in accordance with recognized standards; providing patient care within limits defined by delegated tasks. Tasks performed under direct supervision of Registered Nurse or Physician.

Qualifications:

1. Must be a CNA, EMT, or NT
2. Require current CPR and must maintain without lapse.
3. Must have basic EKG and/or arrhythmia course within six months of employment.

Med Surg - Charge Nurse - RN - FT 7p to 7a shift    

Summary: The Charge Nurse (RN) is responsible for utilizing the nursing process in providing the health care needs of assigned patients.

Qualifications:

Tasks performed under direct supervision of a Charge Nurse/Nurse or Physician. Only Certified Nurse’s Aides may work in the hospital environment.

Qualifications:

1. Must be a Certified Nurse Aide or entered into Certified Nurse Aide program and completed the first 16 hours of course work (proof must be provided)
2. Must complete Certified Nurse Aide program within 6 months of hire.
3. Maintain certification without lapse.
4. Prior clinical experience preferred.
5. Current CPR or have certification within 3 months of hire.
6. Ability to care for and about others.
7. Must be of good physical and mental health.
8. Good assessment and observation skills.
9. Possess adequate speech and hearing to communicate well.

Surgical - RN-Full Time Pre-op and PACU. Monday thru Friday 7a to 4p, no weekends.

Summary: The RN assumes responsibility for the anesthetized patient under the supervision of the anesthesia personnel; directing, planning, and evaluating on-going patient care using the nursing process. Actively participates in quality initiatives and improvements that focus on patient care and nursing practice. Identifies and manages emergency patient situations efficiently. Must have knowledge of Life Support, Anesthesia, Pain Medications and Antibiotics.

Qualifications:

1. Licensed with a Texas or Compact RN Licensure Required.
2. Require current BLS, ACLS and PALS, must maintain without lapse.
3. Requires strong organizational and time management skills.
4. Ability to function in a team environment with positive, enthusiastic demeanor.

Surgery - CST-Full Time for Orthopedic and Neuro cases. Monday thru Friday 7a to 4p, no weekends.

Summary: The CST assists surgeon during operative and invasive procedures, ensure suite is adequately prepared for procedure, monitor PAR level of all surgical instruments and supplies, cleans and sterilizes all surgical instruments, and participates in departmental staff meetings and performance improvement activities.

Qualifications:

1. High School Diploma or equivalent.
2. One year Operating Room Technician program required.
3. CST certification from an accredited technical school required and must maintain without lapse.
4. Minimum one year Scrub Technician experience in Operating Room preferred
5. Require current BLS and must maintain without lapse.

Surgery - Central Sterilization Technician

Summary: The Noncertified Surgery Technician will ensure operating suite is adequately cleaned according to AORN standards between cases and at the end of day, monitor PAR level of all surgical instruments and supplies, clean and sterilize all surgical instruments, and help maintain a clean and safe department.

Qualifications:

1. High school diploma or equivalent.
2. A formal training program after high school (preferred), Certified Nurse Assistant (CNA) is acceptable.
3. Minimum of six months work experience preferred.
4. Require current CPR and must maintain without lapse.
5. Must complete Central Sterilization Certification within one year of hire and must maintain without lapse.

Welcome (Assisted Living) LVN - FT & PRN Days

Summary: The Welcome Nurse Coordinator is responsible and accountable for the delivery of nursing services in the assisted living facility.

Qualifications:

1. Licensed and currently registered as a Vocational Nurse in the state of Texas or possesses a temporary permit to practice vocational nursing in Texas.

Speech Language Pathologist-Audiologist

Speech Therapist/PRN Position Available

Summary:  Conducts evaluation, rehabilitation, and training in the areas of speech and language disorders. Works under the general direction of the policies and procedures of the hospital, and, as appropriate, under the policies of contract facilities and the referring physician.  Under the direct supervision of the Physical Therapy Director.

Qualifications: 

1.  Must possess a current State Board of Examiners Speech-Language Pathology and Audiology license
2.  1 - 2 years of experience preferred

Social Worker - FT

Summary:
Counsels and assists patient and their families with personal and environmental difficulties which predispose illness or hinder the patient from receiving maximum benefits from medical care. Services to be provided in an inpatient hospital setting or long-term care setting.

Principle Duties/Responsibilities:
1.   Assess the needs of the patient/family.

Occupational Therapist - Full Time

Summary: The Occupational Therapist will work under the direct supervision of the Therapy Manager. The occupational therapist plans and conducts medically prescribed rehabilitation and training of physically limited and disabled inpatients. The Occupational Therapist supervises the activities of the certified occupational therapist assistants and participates in the plan of care with other disciplines to retrain or inpatients, to facilitate maximum independent functioning and return inpatients to their homes or other environments in the best condition possible.

LVN Night Charge Nurse - FT

Summary: The primary purpose of your job position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by nursing assistants. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing Services or Nurse Supervisor to ensure that the highest degree of quality care is maintained at all times.
Delegation of Authority. As Charge Nurse you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

Qualification:

1. Licensed LVN in the State of Texas
2. Long term care experience is a plus.
3. Must have BLS

PRN Charge Nurse:   LVN

Summary: Provide direct nursing care for rehab patients and long-term residents and supervise the day-to-day nursing activities performed by nursing assistants and ensure that the highest degree of quality care is maintained at all times.
 
Qualifications:

1. State Licensure Required or Compact Licenses

CMA - Certified Medication Aide Full Time - Day Shift (3-Day weekend off every other week)
