EMS

E.M.T. Basic - PRN

Summary: To provide professional care and transport of the sick and injured to hospital or other health facility in emergency and non-emergency situations. May perform a variety of basic life support skills in the pre-hospital and hospital setting. Perform Security functions as required.

Qualifications:

1. High school graduate or equivalent.
2. Certified by the Texas Department of Health as an EMT-Basic with BLS certification.
3. Must be over 18 and possess a Texas driver’s license with a clean driving record.
4. Two years' experience in an EMS position preferred.

Home Health 

RN Casemanager - Full Time

Summary: The Registered Nurse (RN) is a professional member of the health care team who provides skilled nursing care to patients in compliance with the Nurse Practice Act and within the policies and procedures of the agency, upholding standards of nursing practice.  Keeps abreast of current techniques and innovations to meet special needs of the patients and staff.  Is responsible for monitoring, altering and coordinating the services the patient receives to promote optimum care. Physical Therapy

Speech Language Pathologist-Audiologist

Speech Therapist/PRN Position Available

Summary:  Conducts evaluation, rehabilitation, and training in the areas of speech and language disorders. Works under the general direction of the policies and procedures of the hospital, and, as appropriate, under the policies of contract facilities and the referring physician.  Under the direct supervision of the Physical Therapy Director.

Qualifications: 

1.  Must possess a current State Board of Examiners Speech-Language Pathology and Audiology license

Director of Nursing - FT

Summary:  Must have a current State of Texas RN License and or a Compact License to practice nursing as an RN. Prefer an experienced Director of Nursing to organize and oversee all nursing operations of the facility. You will create a collaborative and efficient workplace with attention to quality standards and policies.
1. Directing, overseeing and evaluating all nursing personnel (registered nurses, LPN etc.)
2. Guiding staffing procedures
3. Setting objectives and long-term goals for the nursing department

Responsibilities:
1.  Direct, oversee and evaluate all nursing personnel (registered nurses, LPN/LVN, CMA & CNA etc.)

LVN – FT Quality Assurance/Infection Control & Wound Care Nurse, with some Administrative Duties -
Will back-up charge nurses as necessary

Summary: The QA and Wound Care Nurse works closely with the Director of Nursing to ensure that residents are evaluated, monitored and treated to prevent or care for sores, tears or wounds that may be present upon admission or during the course of care. This position also leads, coordinates and implements the facility’s QA & infection control efforts, along with providing assistance during to the admission process or to charges nurses as needed. Some Administrative Duties will be required as well.

Qualifications and Competencies:
1. Current licensure as a Licensed Practical Nurse (LPN/LVN) in the state of Texas.
2. Eventual ability to demonstrate knowledge, technical proficiency and competency in facility, department and job specific policies, procedures, practices and duties that are needed and required for the patient populations treated.
3. Possess the physical, mental, sensory and other required abilities outlined in the Job and Work Environment Requirements.

QA/Infection Control:
• Plans, develops, organizes, implements, evaluates, coordinates and directs the infection control program in compliance with CDC, OSHA, State and other applicable rules and regulations.
• Develops, adapts and implements internal infection control policies and procedures as necessary, including decontamination and sterilization activities performed in the facility.
• Measures and assists quality improvement personnel and/or committees in measuring the effectiveness of infection control initiatives.
• Recommends precautionary aseptic techniques and corrective actions to nursing and facility leadership and other applicable committees to reduce patient’s and employee’s exposure to blood/body fluids and other infectious sources.
• Conducts surveillance of infection controls practices and procedures to ensure all personnel are implementing our standard operating procedures in regards to infection control.
• Visit isolated residents as necessary to ensure that established isolation precautions and aseptic technique are followed.
• Monitors and tests sterile equipment, monitors shelf life of sterilized equipment and develops and follows procedure for removing infective and contaminated materials from the facility.
• Investigates, tracks, records, reports and recommends corrective actions concerning nosocomial and community-acquired infections.
• Assists and integrates efforts with laboratory, laundry, dietary, biomedical and other applicable functions and services in developing practices that promote infection control.
• Assists in reporting all reportable diseases to county and state health departments.
• Other infection control duties as assigned or necessary to undertake.

Wound Care:
• Completes comprehensive skin assessments upon admission or during other regular intervals as directed, and recommends appropriate prevention and/or treatment interventions.
• Interacts with patients to evaluates and initiate a plan of care that promotes patient's progressive understanding and active participation in the wound healing and rehabilitation process.
• Makes recommendations regarding support devices or other care environment changes to assist in the pressure relief of wounds, in addition to products for the treatment and management of wounds.
• Serves as a resource for other nursing or aide staff in providing specialized wound care individualized for each resident’s needs.
• Develops educational programs, as directed, to improve staff support of wound care best practices on a facility-wide basis.
• Performs other wound care duties as assigned, or as necessary.

Other Duties:
• Completes patient electronic or other charting, and keeps sufficient records and documentation as required by applicable standards or as is necessary.
• Participates in inter-disciplinary care and quality teams and committee tasks, initiatives and meetings.
• Assists other nursing staff in evaluating potential admissions, and assists in patient admission and ensures all intakes procedures are completed on a timely basis.
• Provides support to charge nurses and other nursing staff as requested or required.

RN or LVN Charge Nurse – FT

Summary: The primary purpose of your job position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by nursing assistants. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing Services or Nurse Supervisor to ensure that the highest degree of quality care is maintained at all times.

Qualifications/Experience/Requirements:

1. Graduation from an accredited nursing school and a current, valid license or registration (RN or LPN/LVN) in the state of Texas.
2. Nursing experience in a long-term care facility is preferred.
3. Supervisory experience is preferred
4. Effective communication and interpersonal skills are necessary.
5. Functional literacy in English is required.

Charge Nurse Responsibilities:
 
1. Administration of Medication and Treatments
2. Notes and carries out physician and nursing orders
