Coryell Health Medical Clinic

Health Coach - Full Time

Summary: The Health Coach is responsible for the provision and/or oversight of wellness care for Coryell Health Medical Clinic patients in accordance with the Texas Nursing Practice Act. This person is responsible for overseeing and coordinating health services with members of the CH team and other community providers in order to provide preventative health services routinely.

Qualifications:

1. Currently licensed as a Licensed Vocational Nurse, CNA or MA in the State of Texas.  Certified health educators may also be considered with an associate’s or bachelor’s degree.
2. Prefer two years of experience in a clinical setting.
3. Must be able to work independently and part of a team, possess strong organizational and time management skills and excellent verbal and written communication skills.
4. Must have knowledge of standard record keeping procedures (i.e. Progress notes, HIPPA guidelines).
5. Must be able to facilitate communication and problem solving with patients, families, staff physicians and administration.

Admissions Clerk - Full Time

Summary: Operates switchboard, patient registration (ER, impatient, outpatient) and various other business office functions.

Qualifications:

1. High School Diploma
2. Preferred previous PBX and data entry experience
3. Good communication skills and customer relations required
4. Clerical skills necessary
5. Familiarity with hospital services and medical terminology.

Referral Clerk - Full Time

Summary:  Responsible for processing referrals and obtaining authorizations for patients.

Qualifications:

1. High School diploma or equivalent.
2. Typing and office equipment experience to include computer operations.
3. Previous experience with medical office procedures preferred.
4. Neat, organized, able to work independently and interdependently, welcoming to the public.

Responsibilities:

1. Handles referrals and prior authorizations for all providers in the office.
2. Provides accurate demographics, patient chart notes, labs, and x-ray reports to physician offices or hospitals that patients are referred to.
3. Provides patients with all necessary information regarding referrals to all entities.
4. Communicates with all patients regarding referral process.
5. Scan each referral and or authorization for patients being referred.
6. Communicates with providers regarding authorizations that are denied or a patient has declined.
7. Document referral process on Referral spreadsheet.
8. Maintain confidential patient information.

Clinic LVN - Monday thru Friday 8a to 5p Full Time
          
Summary: Performs assigned nursing duties, including assessing, planning, implementing and evaluating patient care, under the direct supervision of an RN/Physician/Midlevel Provider.  Also performs a wide variety of patient care activities including, but not limited to, maintenance of clinical record/documentation, administering medications, giving injections, and taking vital signs.

Qualifications:

1. Ability to facilitate communication and problem solving with patients, families, staff physicians, and administration.
2. Basic Life Support
3. Licensed in the State of Texas as a Licensed Vocational Nurse or possesses a temporary permit to practice vocational nursing in Texas. Nurse with temporary permit (GVN) has 75 days to take the NCLEX after they receive the Authorization to Test per Texas Board of Nursing. If GVN fails NCLEX the temporary permit is revoked by the Texas Board of Nursing and job status at Coryell Medical Clinic will be demoted to position and pay prior to GVN status (nurse tech/CNA) or may result in termination.
4. Prefer one year or more of nursing clinical experience.
5. Prefer the following skills:  the ability to multi-task, problem solving, organizational skills, experience using an Electronic Medical Record.

Culinary

Cook/Cook Assistant - Full Time

Summary: Under the direction of the Food Service Supervisor, prepares food as planned on cycle menu, using standardized recipes. Correctly portions and serves food. Uses accepted storage techniques. Maintains a sanitation standard, follows safety regulations, keeps record as required. Supervises and trains Food Service staff members, upholds policies of Coryell Health and its long term care facilities.

Qualifications:

1. High School Diploma or equivalent.
2. Culinary Degree a plus
3. Must be Food Protection Management Certified.
4. Must have 3 plus years institutional food service experience, including knowledge of food production, therapeutic diets, and sanitation.
5. Must be able to work flexible schedule, including weekends and holidays, depending upon the needs of Coryell Health and its long-term facility as directed.
6. Must be able to read, write, and communicate orally in English.

Coryell Health Home Health

Home Care Attendants: PCA-PRN Positions Available - Gatesville area/hours available

Summary:  Provides personal care to patients in their home within the scope of their abilities and in accordance with the policies set forth by the agency under the supervision of a registered nurse.  Attendants are needed to cover the Gatesville, and Evant areas.

Qualifications:

1. Must have empathic/caring attitude in caring for patients.
2. Must have dependable transportation with liability insurance.
3. Proficiency in managing confidential materials and information with integrity.
4. Good observation and communication skills.

Diagnostic Imaging

CT/XR Staff Radiological Technologist - PRN

Summary: Performs in accordance with physician prescription those duties directly involved with a variety of technical procedures applying radiation or other forms of energy used in diagnostic imaging. Assists in collecting and reporting data to insure performance improvement. Utilizes proper Radiation Safety procedures. Develops films, transports patients and completes clerical assignments.

Qualifications:

1. Graduate of approved and accredited school of radiological technology.
2. Certified with the State of Texas as an MRT.
3. CT Experience Necessary.
4. ARRT registered or registry eligible.
5. Some computer skills helpful.

EMS

EMT-Paramedic - Full Time

Summary: Non-supervisory position in EMS department. Participating as a member of a MICU team or as an individual to provide patient focused delivery of care, treatment and safe transport for emergency and non-emergency patients in a clinically safe and professional environment, utilizing enhanced skills and regulated best practice when required to do so, always adhering to the policies and standards inherent within the Coryell Health EMS (CHEMS).

Qualifications: (Knowledge)

1. High School Diploma/GED
2. Hold a current State of Texas certification or license as a Paramedic.
3. Knowledge of the information and techniques needed to diagnose and treat a human injury, illness and disease.
4. Knowledge of the operation of medical equipment utilized in pre-hospital emergency care.
5. Good physical condition with the ability to pass a physical performed by a physical and a physical agility test as required.
6. Current Basic Cardiac Life Support Certification, Advanced Cardiac Life Support and Pediatric Advanced Life Support not more than six months past expiration.
7. Complete Field Training Program and probationary requirements.

Material Management

Central Supply Clerk - Full Time

Summary: Responsible for stocking shelves and maintaining the overall appearance of Central Supply. Must be able to check ER, Med Surg, ICU, Welcome and the Meadows and stock the floors quickly and efficiently. Must be able to work independently and have an eye for detail and what needs to be completed.  Must have medical supply knowledge; you will be the first one that customers will come to when entering the department.

Qualifications:

1. High School diploma or equivalent.
2. Able to manually check departments for items needed and accurately fill the supplies from the floor stock sheet.
3. Able to check in deliveries that arrive from various vendors via UPS, FEDEX, FEDEX GROUND and other carriers. Remember to find the Purchase Order and the Request to Purchase for each item delivered. Compare product invoices to items actually received to ensure that shipments are correct.
4. Assist internal and external customers when they enter the department
5. Stock shelves and bins
6. Clean shelves and aisles

Administrative Nursing Assistant - Full Time 

Summary:
The Administrative Nursing Assistant will maintain payroll records and reporting procedures and organize and maintain employee competency records for all hospital nursing departments. He/She will report to the Chief Nursing Officer and Nurse Directors on overtime, attendance and tardiness.  He/she will assist with other tasks as assigned to maintain the needs of the nursing departments, including organizing meetings and taking minutes. The Administrative Nursing Assistant will also maintain harmonious inter and intra departmental relationships.  He/She will maintain strict confidentiality concerning personnel and other sensitive information.

Qualifications:

1. High school education or equivalent; (some college preferred) and experience.
2. Be skilled in management of interpersonal relationships, organization, and coordination of tasks and personnel with the ability to communicate effectively (verbally and in writing).

ICU RN/Registered Nurse: Full Time - 7p to 7a

Summary: Demonstrates an ability to competently provide individualized goal directed nursing care to ICU/CCU and Emergency Department patients through the use of the nursing process from admission to discharge.

Qualifications:

1.    RN Licensed and currently registered as a Professional Nurse in the state of Texas or possess a temporary permit to practice professional nursing in the State of Texas while awaiting receipt of license.
2.    Required to meet qualification criteria during hospital and nursing unit orientation periods.
3.    Previous ED/ICU experience desirable.
4.    Required current BLS and ACLS Certification.
6.    Must have PALS within one year of employment and must maintain without lapse.
7.    Must have NIHSS certification within six months of hire and maintain without lapse.
8.    Must have TNCC certification within eighteen months of hire and must maintain without lapse.
9.    Must have basic EKG and/or arrhythmia course within six months of employment.
10.  May satisfy requirements with documentation from previous experience.

Summary: The RN/Registered Nurse is primarily responsible for utilizing the nursing process and nursing standards in providing health needs to the patient in the Med Surg department.

Qualifications:

1.    RN/Registered Nurse licensed and currently registered as a Professional Nurse in the state of Texas or possesses temporary permit to practice Professional Nursing in the State of Texas while awaiting receipt of license.

Ward Clerk: Days PRN

Summary: Performs multiple clerical duties.  Prioritizes and transcribes/types orders working closely with nursing.  Maintains confidentiality at all times.  Assembles, organizes and maintains the patient medical record during hospitalization under the direct supervision of the Registered Nurse.

Qualifications:

1. High school graduate or equivalent.  Medical Terminology Course desired.
2. Former experience preferred
3. Must be able to read, write and communicate in English
4. Basic computer skills required.  Ability to type.

Assisted/Independent Living at The Oaks

CMA: Full Time and PRN Days and PRN Evenings

Summary: The CMA is to assist in the administering of medications to residents as ordered by the attending physician, under the direction of the attending physician, nurse supervisor, or the Director of Nursing Services. The administration of medications shall be in accordance with established nursing standards, policy and procedure of this facility, and the requirements of the state.

Qualifications:

1. Must be a Certified Nurse Aide & maintain without lapse.
2. Must be certified by the state of Texas as a Certified Medication Aide and maintain without lapse.
3. Prior clinical experience preferred.
4. Requires current CPR and must maintain without lapse.
5. Must be able to write legibly.
6. Must be of good physical and mental health.
7. Good assessment and observation skills.
8. Possess adequate speech and hearing to communicate well.

CNA: PRN

Summary: The CNA/NA will care and treat patients/residents in a manner conductive to their safety and comfort. Perform duties in accordance with established methods and techniques in conformance to recognized standards. Provide patient/resident care within limits defined by delegated tasks. Tasks performed under direct supervision of a Charge Nurse/Nurse or Physician. Only Certified Nurse’s Aides may work in the hospital environment.

RN/LVN MDS Coordinator - Full Time

Summary: The primary purpose of your job position is to coordinate delivery of services to managed care and Medicare residents in collaboration with the facility’s team members. The Case Manager monitors and documents the cost effectiveness of treatment provided, facilitates and coordinates the admission and discharge process, serves as the resident and family advocate and acts as a liaison to insurance and medical management professionals. 
Delegation of Authority. As Case Manager you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.
