Dialysis Social Worker – FT

Hours:

Full Time:  8am to 5pm Monday - Friday

Job Description:

Performs assigned social work/social services duties, including assessing, planning, implementing and evaluating patient care.  Also performs a wide variety of patient care activities including, but not limited to, maintenance of clinical record/documentation, discharge planning, referrals, service linkage, verification of insurance benefits, and coordination for patient care post hospitalization.  Tends to the day to day issues and communicates needs and issues with patient, patient’s family as appropriate, and other staff.  Actively participates in outstanding customer service and accepts the responsibility in maintaining relationships that are equally respect to all.

Job Qualifications:

Qualifications:
  1. Ability to facilitate communication and problem solving with patients, families, staff physicians, and administration.
  2. Basic Life Support
  3. Licensed in the State of Texas as a social worker
  4. Prefer two years or more of medical experience with at least one year in ambulatory care setting.
  5. Prefer the following skills:  the ability to multi-task, problem solving, organizational skills, and experience of using an Electronic Health Record.
Typical Physical Demands:  
Requires a full range of body motion.  Occasionally lifts and carries items weighing up to 50 pounds.  Also requires standing and walking for extensive periods of time. Requires working under stressful conditions. Be able to impart oral information to patient or to the public (i.e. visitors, patient’s family) and must convey detailed or important information to other workers and physicians accurately, loudly, or quickly.  It requires the ability to apply common sense understanding to carry out instruction furnished in written or oral form.
Typical Work Environment:
Will be occasionally exposed to communicable diseases, blood, and other bodily fluid borne diseases, toxic substances, ionizing radiation, medicine preparations, and other conditions common to a clinical environment.
