Health Facility Compliance
If you have a question or concern about Coryell Health’s compliance with local, state or federal laws, we encourage you to visit the Texas Health & Human Services website for specific contact information related to your concern. We also encourage you to notify a member of the Coryell Health team with your concerns. We take compliance very seriously.
Filing A Consumer Complaint
If you need to file a complaint or want to learn more about protecting your privacy and reporting fraud or abuse, we encourage you to visit the Texas Attorney General’s information page on this topic. From there, you can learn more about your rights and start the process of filing a consumer complaint.
Medical Records Requests
Coryell Health is committed to providing patients with easy access to their medical records while maintaining confidentiality of patient information.
To request copies of your medical records, please download, complete and sign the Release of Information form. Return this form to the Health Information Department any of the following ways:
