Home health care provides a wide range of health care services in the home for an illness or injury. Home health care helps patients regain their independence and become as self-sufficient as possible from the comfort of home.

SERVICES HOME HEALTH PROVIDES UNDER THE DIRECTION OF A PHYSICIAN:

  • Skilled Nurses that are able to assess and treat medical conditions and help with an individualized care plan.
  • Home Health Aids can assist with everyday personal care like bathing and exercising. Light housekeeping and meal preparation are also available with some plans.
  • Physical & Occupational Therapists will help increase mobility, improve coordination, and regain independence.
  • Licensed Medical Social Services will assess, evaluate, and recommend community programs

Who Qualifies for Home Health Care?

Medicare/Medicaid – An eligible Medicare/Medicaid beneficiary who is homebound qualifies. Medicare defines homebound as requiring considerable effort and assistance to leave home. Skilled services must be required on an intermittent basis.

Insurance coverage: Some insurance may pay for skilled services in the home under a physician’s care, and will be determined on an individual basis.

Private pay: Persons wishing to get home health services may receive any of the listed services by paying for it themselves. This can also include light housekeeping, shopping, errands, standby assistance with ambulation or exercise, and assistance with oral medications.

Waiver programs: We offer attendant services under Primary Home Care and attendant plus skilled services under the Star Based Wavier. Qualification for these programs are determined by the Texas Department of Aging and Disability Services.

How do I begin Home Health Care?

Contact us for more information and to schedule an evaluation in your home. We also accept referrals from physicians, hospitals, patients, and concerned individuals.
